In Meniere's disease, vertigo lasts between 20 minutes and 12 hours and does not stop when stationary. Also, hearing loss, fullness, and ringing are seen in the affected ear.

  • Although the exact underlying mechanisms are not known(genetic, viral, autoimmune, allergic?), symptoms are caused by increased inner ear(endolymph) fluid pressure.

  • Middle-aged women are more frequently affected (similar to migraine). In %80 cases, only one of the ears is affected.

  • At least two episodes are necessary for suspicion. A documented hearing loss during an episode makes the diagnosis definitive.

  • Preventive measures (limiting caffeine/alcohol/chocolate/excessive salt intake, quitting smoking, and reducing stress) can help most of the patients.

  • Betahistine and diuretics can be used for 3-6 months to decrease the frequency of attacks.

  • Attacks are treated with vestibular suppressants, steroids, diuretics, and oral betahistine.

  • Intratympanic steroid injections can offer a hearing-preserving non-surgical option. If the hearing is non-serviceable, intratympanic gentamycin injections can be preferred for a non-surgical vestibular ablation.

  • If these measures can not control the debilitating symptoms, surgical options such as endolymphatic sac decompression, vestibular neurectomy, or labyrinthectomy may be considered.
