Non-infected fluid collection in the ear is called serous otitis media. This condition can cause mild earache and decreased hearing and is usually due to Eustachian tube dysfunction, often linked to adenoid/sinus infections, gastric acid reflux or allergies.

Many children experience temporary hearing loss or earache due to colds. Infections can trigger an increase in mucoid secretion and swelling of the immune tissues in the adenoids and Eustachian tube, leading to fluid collection in the ear. This fluid suppresses sounds, causing hearing loss in children. The eardrum appears opaque and does not move normally when pressurized air is applied. This condition is the most common cause of conductive hearing loss in children.

A chronic non-infected (but thickened) fluid collection that completely fills the middle ear. The membrane appears to have a dull appearance, thickened and vascularized due to inflammation.
